CMS Accepts 150+ Orgs Into ACCESS Chronic-Care Model
CMS has approved more than 150 providers and digital health firms into the ACCESS model, a value-based structure paying $7.50 to $35 per member per month for managing Medicare beneficiaries with chronic conditions across mental and physical health.
